Please print and fill out our New Patient Form before your initial appointment to avoid any delay when you arrive at our clinic. Please provide our team with your medicare card and health insurance details. 
For all your appointments
Please ensure that you bring a valid referral from your medical practitioner. Please ensure you bring results and copies of all your previous investigations such as blood tests, biopsy results, audiograms and Xrays for example ultrasound, CT or MRI scans. Cancellation of appointment If you do need to defer or cancel your appointment with Dr Palme, please inform our team at your earliest convenience in order to arrange an alternate date. Patients Requiring Surgery All patients scheduled for surgery will fill out their paperwork with Professor Palme at the time of consultation. Our Team will be glad to assist you in completing your forms, provide financial consent and go through all the steps necessary for your surgery. |
Please inform our team regarding any significant past medical history including any previous surgical procedures and regular medications. This importantly includes any medications that may cause problems with blood clotting. Medication that may cause excessive bleeding include ASPIRIN, NON STEROIDAL ANTI-INFLAMMATORY DRUGS ( ie. Voltaren, Nurofen ), PLAVIX, ASASANTIN, PRADAXA and WARFARIN. Please also inform our team whether you take any alternative medication including GARLIC, VITAMIN E Supplements and GINSENG. It is very important that you bring all relevant results such as blood tests, biopsy results and importantly Xrays such as Ultrasound, CT and MRI scans on the day of your surgery.
